My name is Dr Pavi Pasupathy and I will be your anaesthetist for your upcoming procedure.

I am a specialist Anaesthetist at Children’s Hospitals Westmead and Randwick, Prince of Wales Hospital and various other private hospitals across Sydney. 

After completing my medical degree at the University of New South Wales, I undertook specialist anaesthetist training at Westmead Hospital. I went on to complete Paediatric Anaesthesia fellowships at Children’s Hospital Westmead, and subsequently Hospital for Sick Children, Canada.

My aim is for you to have a positive experience for your surgery with as little pain and discomfort as possible after the procedure.

The hospital will commonly have you complete a similar questionnaire prior to your admission for their own records – this is not provided to your Anaesthetist.

To be able to provide the best anaesthetic care for your upcoming procedure, could you please complete the following questionnaire.

The questionnaire will take approximately 10 minutes to complete and requests information including any current medical conditions and medications.

Your completed questionnaire will be forwarded to me, and I will contact you directly if further information or clarification is required, or if you have noted a particular concern and requested that I contact you.
